Parent Input Form

Over the past few weeks I have been preparing to collect information to build next school year's class placements. This is an important process and one that we take very seriously. As we begin to develop our class lists, we take into account our guidelines while keeping in mind that we need to create a balanced classroom for our students. Please know that this challenging process involves many variables such as gender balance, academic mix, personality and learning styles, peer relationships, class size and special needs. The objective is to place all students with a teacher in a balanced class that will support their learning and growth. At Maplewood, we are fortunate to have an exceptional group of teachers and staff to work with our students and families.

We will be providing families with the optional opportunity to fill out a ‘Parent/Student Information Form.’ This form will not be used for requesting a teacher, but will give you a chance to share valuable information about your child. We will not be honoring specific teacher requests. The form will be sent home with your child upon request. Please call the main office at (814)337-1659, email the main office at, send in a note or ask your child’s classroom teacher through DoJo or email to have one sent home. You can also print an input form out from our website at   

After you have completed the form, please return the form to your child’s current classroom teacher no later than Friday, May 28, 2021. Final class lists will be completed over the summer. You will be notified of your child’s homeroom placement on August 19, 2021 by logging onto PowerSchool.

This is an important process that needs to be fair, consistent and student focused. Our staff will work hard to ensure that your child is placed appropriately using the criteria previously stated. Thank you in advance for your cooperation!
