New Student Registration

We are always excited to welcome new students! Our office is open Monday - Thursday from 7:00AM-4:00PM for you to stop in and register. To enroll your child please bring the following with you to register:

• Proof of age (birth certificate)
• Immunizations / short records
• Proof of residency (must be 1 of the following: mortgage, lease, deed, rental agreement, property tax bill, or closing disclosure statement)

You will follow the other forms out with us in the office or print the packet out linked on this page to the right to have them done prior to your visit:

Maplewood Elementary School is gathering information to determine the number of children in our attendance area who will be old enough to attend Kindergarten during the 2026-2027 school year.

You can start the registration process by calling the main office at 
(814) 337-1659. We will mail you a student registration packet that can be returned to our main office.

Please bring completed forms to the office at Maplewood Elementary School between the hours of 7:30am and 3:45pm on any day that school is in session.

Enrollment Forms

Be sure to bring the following documents to complete your registration

1. Birth Certificate
2. Immunization Records
3. Government ID (drivers license, military id, passport, etc.)
4. Proof of Residency (mortgage, lease, deed, rental agreement, property tax bill or closing disclosure statement
